I suggest going through the piece you are working on, and taking each note one at a time. Here are some things to remember about tone--keep the head up, shoulders down, jaw down, tongue down (that will definitely hinder your air!), and just try to get that particular note to sing at any dynamic leve...

Going to the top D is about as far as anyone will go; unless of course you want to get into some zany extended technique, 20th, 21st century type stuff. Practicing those high notes can be damaging to the embouchure. Personally, I wouldn't do the scales 3 octaves. The only scales I practice 3 octaves...
