I would make sure your pic is in good adjustment. Maybe there are some leaks. Once you have determined that the instrument is fine, you just have to keep practicing. Try keeping your head up and back straight to open your airways as much as possible. Not enough air speed can make you sound flat. Try practicing standing up, that might help with pitch as well.
