They are pretty uncommon, but useful.

There was recently a discussion on the Galway Flute Chat about them.

Anyway, they work by adding an extra rod on the body joint that meets a connector on the footjoint.

It basically looks like you have an extra Ab key.

AHA!!!!!

I found a picture!

It is a used Haynes. Open hole, inline G, low Bb [yes, B flat] footjoint, and a lever to activate the B key.
